??Luther Manor is seeking a Full-Time Hospice Registered Nurse

The Hospice Registered Nurse is responsible for fulfillment of patient's total needs including assessment, medication management, documentation, supervision of other nursing personnel and performing as the Care Coordinator. ?

Essential Job Functions:

  • ??Participate as part of the core team providing care and services and be designated to coordinate the implementation of the plan of care.

  • ?Participate in developing and revising written patient care policies and procedures.

  • ?Participate in the development, review and updating of a patient's plan of care.

  • ?Actively participate in the hospice's quality assurance program.

  • ?Assess patient and family needs as directed by the care coordinator.

  • ?Promptly notify the Care Coordinator or Hospice Director of any change in patient status that suggests a need to update the plan of care.

  • ?Provide services consistent with the patient plan of care.

  • ?Provide education and counseling to the patient and, as necessary, to the patient's family, consistent with the plan of care.

  • ?Regularly assess the patient's nursing needs, implanting the plan of care provisions to meet those needs and re-evaluating the patient's nursing needs.

  • ?Shall be part of a hospice program which ensures nursing services are available twenty-four (24) hours a day.

  • ?Supervision and teaching of other nursing personnel, including licensed practical nurses, home health aides and hospice aides and their provision of care and documentation.

  • ?Evaluation of the effectiveness of delegated acts performed under the registered nurse's supervision.

  • ?Nursing services must be directed and staffed to assure that the nursing needs of patients are met.

  • ?Patient care responsibilities of nursing personnel must be specified.

  • ?Services must be provided in accordance with recognized standards of practice.

  • ?Maintains current knowledge base of medications, administration and monitoring.

  • ?Ongoing observations, assessment and documentation of patient and family needs.

  • ?Notification of physician with change of condition or needs.

  • ?Communicates status to patient and family as needed.

  • ?Completes all required documentation.

  • ?Maintains honest, open and professional communication with others.

  • ?Provides consistent fair disciplinary action when appropriate.

  • ?Practices universal precautions.

  • ?Demonstrates knowledge and skill in the use of medical equipment, i.e. suction, oxygen, nebulizer,?etc.

  • ?Responds per phone to patient/family call within fifteen (15) minutes.

  • ?Participates in continuing education opportunities.

  • ?Abides by all Luther Manor Hospice Care Policies and Procedures as outlined in the employee

  • ?handbook.

  • ?Shall respect the right of the resident to select and refuse services.

  • ?Other duties as assigned

  • ?Current Wisconsin RN licensure.

  • ?Mathematical ability to determine medication and fluid calculations.

  • ?Ability to speak, hear, and write legibly to communicate with patients, families and staff.

  • ?Ability to organize tasks and function independently.

  • ?Ability to maintain a good attendance record.

  • ?Good personal hygiene.

  • ?Ability to stand or walk, push or pull, bend or twist, sit, kneel or crouch, and reach high or low levels.

  • ?Ability to use arms and hands to push, pull, grasp or write.

  • ?The physical strength to move or assist residents as needed following their plan of care.

  • ? Must be able to push, pull or lift 50 pounds per Luther Manor Occupational Health Nurse.

  • ?Ability to tolerate exposure to blood and body fluids.

  • ?Tolerant of hands being exposed to water, soap and protectant gloves.

  • ?Tactful, empathetic, and courteous.

  • ?Free of police record related to abuse or misappropriation of property.

  • ?Must have a valid Wisconsin driver's license and a good driving record.

  • ?Must have auto insurance with Luther Manor minimum coverage requirements.
