Hospice RN Weeknight On-Call

James River Hospice is actively seeking a compassionate and experienced Hospice RN Weeknight On-Call to join our growing team in Richmond, VA!

In this role, you will provide after-hours hospice nursing care, respond to patient needs, complete assessments and admissions as needed, and coordinate with our interdisciplinary team to ensure patients and families receive exceptional care.

Schedule: Monday - Thursday 5 PM- 8 AM

Salary Range: $85,000 - $105,000

Key Responsibilities

• Respond to after-hours patient calls and provide nursing visits as needed
• Complete initial patient and family assessments to determine hospice needs
• Perform hospice admissions as assigned
• Develop and update individualized plans of care
• Provide pain and symptom management
• Monitor, document, and report changes in patient conditions
• Collaborate with physicians and the interdisciplinary team
• Obtain physician orders as needed
• Coordinate care with LPNs and hospice aides
• Provide compassionate support and education to patients and families
• Complete required clinical documentation

Qualifications

• Active Registered Nurse (RN) license in Virginia
• Minimum of one year of hospice nursing experience required
• Graduate of an accredited nursing program
• Valid driver's license and reliable transportation
• Strong assessment and communication skills
• Ability to work independently during overnight on-call hours
