What you will do as a Medical Social Worker:
- Conduct comprehensive psychosocial assessments and develop individualized plans of care.
- Provide counseling, emotional support, crisis intervention, and education to patients, caregivers, and families.
- Collaborate with the interdisciplinary team to develop, implement, and evaluate patient care plans.
- Coordinate community resources and assist with financial, legal, caregiver, and psychosocial needs.
- Assist patients and families with advance care planning, including advance directives and end-of-life wishes.
- Advocate for patients while collaborating with physicians, hospitals, long-term care facilities, and community agencies.
- Participate in family meetings and interdisciplinary care conferences.
- Complete timely, accurate documentation in accordance with Medicare regulations and agency standards.
- Support hospice admissions, discharges, and transitions of care as needed.
- Participate in quality improvement initiatives, continuing education, and organizational programs that support excellent patient care..
What we are looking for:
- Master's Degree in Social Work Required.
- Two years of social work or counseling experience, including at least one year in a healthcare setting.
- Hospice or palliative care experience preferred.
- Licensed Clinical Social Worker (LCSW) or hospice/palliative care certification preferred.
- Experience with psychosocial assessments, counseling, case management, and crisis intervention.
- Excellent communication, organization, and critical thinking skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Valid driver's license, reliable transportation, and proof of insurance.

Schedule:Monday–Friday, approximately 8:30 AM – 5:00 PM
