Job Description

Overview:

Pathway Mentors support student success, persistence, retention, engagement, and completion through individualized case management, proactive outreach, community engagement, and data-informed interventions. Pathway Mentors serve as a primary point of contact for assigned students and families, helping navigate academic, financial, personal, and social barriers while connecting students to appropriate college and community resources. The position supports strategic enrollment, retention, and completion goals through direct student engagement, program implementation, partnership development, and ongoing data collection, tracking, assessment, and reporting.

Responsibilities:

Student Success, Retention, and Completion

  • Maintain and manage an assigned caseload of students.
  • Provide ongoing outreach, coaching, mentoring, and navigation support through in-person, virtual, phone, text, and email communications.
  • Identify and address barriers impacting student success, persistence, retention, and degree completion.
  • Develop individualized student support plans and intervention strategies.
  • Monitor academic progress and connect students to tutoring, advising, financial aid, wellness services, accessibility resources, and other support services.
  • Conduct proactive outreach to students experiencing academic, attendance, registration, financial, or personal challenges.
  • Foster a sense of belonging and connection to Brookdale through relationship building and student engagement activities.
  • Support students through key transition points including onboarding, first-year experience, transfer planning, and graduation completion.

Program Engagement and Community Outreach

  • Participate in college recruitment, community outreach, and engagement events during daytime, evening, and weekend hours.
  • Build relationships with community organizations, schools, social service agencies, and cultural groups to support student access and success.
  • Collaborate with Student Affairs, Enrollment Services, Academic Affairs, Financial Aid, Advising, and other campus departments.
  • Assist with planning, implementation, and assessment of student engagement initiatives, workshops, and special events.
  • Support student clubs, affinity groups, learning communities, and special populations as assigned.

Data Tracking, Assessment, and Reporting Responsibilities

Student Case Management and Documentation

  • Maintain accurate, timely, and complete electronic case notes for all student interactions.
  • Document outreach attempts, interventions, referrals, follow-up actions, and student outcomes.
  • Track student engagement activity and participation in support programs and events.
  • Maintain confidential student records in accordance with FERPA and college policies.

Data Collection and Reporting

  • Collect, monitor, and analyze student success metrics related to assigned caseloads.
  • Track key performance indicators including:
    • Student contact frequency
    • Outreach response rates
    • Persistence and retention rates
    • Credit completion
    • Registration and enrollment status
    • Academic standing
    • Graduation and transfer outcomes
    • Program participation and engagement metrics
  • Maintain program databases and data management systems.
  • Assist with grant reporting requirements and data submissions, summarizing activities, outcomes, and program effectiveness.
  • Support audits, program reviews, and accreditation-related data requests.

Assessment and Continuous Improvement

  • Evaluate effectiveness of student interventions and support strategies.
  • Use student data and outcome reports to identify trends, risk factors, service gaps, and opportunities for improvement.
  • Participate in assessment processes and implementation of evidence-based retention strategies.
  • Make recommendations for program enhancements based on data analysis and student feedback.

Technology and Systems Responsibilities

  • Utilize the College's Student Information System (SIS), CRM platforms, case management systems, and Microsoft 365 applications.
  • Maintain accurate records in student success tracking platforms.
  • Use Microsoft Excel, Forms, or similar reporting tools to support program assessment and reporting.
  • Support data integrity and compliance with institutional policies and grant requirements.

 

Qualifications:

Education

  • Bachelor's Degree required or an equivalent combination of education and experience from which comparable knowledge and abilities can be acquired.

Experience

  • Three (3) years of experience in higher education, student success, advising, case management, social services, workforce development, or a related field.
  • Experience working with diverse and underserved student populations.
  • Experience with student tracking systems, databases, or CRM platforms.
  • Experience collecting, managing, and reporting student data preferred.
  • Experience interacting with community organizations and social service agencies.

  • Experience creating reports and maintaining records with a high degree of accuracy.
  • Proficiency in Microsoft Outlook, Word, Excel, Teams, Forms, and related technologies.
  • Ability to effectively serve diverse, multilingual, and vulnerable populations; bilingual/multilingual skills preferred.
  • Availability for occasional evening and weekend assignments.
