    Job Description

    Summary The Housing & Rideshare Coordinator provides housing and rental assistance, transportation coordination, and supportive services to eligible clients, with a focus on people living with HIV. This position ensures clients receive timely, compassionate, and appropriate assistance while maintaining program compliance, accurate documentation, and confidentiality.

    Responsibilities Housing & Rental Assistance

    Assess and triage clients requesting rental or housing assistance and determine appropriate programs and services. Verify and document client eligibility in accordance with program and funding requirements. Coordinate financial assistance within approved funding guidelines. Provide referrals, advocacy, and connections to healthcare, behavioral health, housing, and community resources. Support clients with independent living skills, problem-solving, and long-term stability. Respond appropriately to client needs and crises, consulting supervisors or interdisciplinary teams as needed. Maintain complete, accurate, and timely client and service documentation.

    Rideshare & Transportation Coordination

    Coordinate transportation for eligible people living with HIV, particularly newly diagnosed or re-engaged clients. Verify eligibility and schedule, confirm, and modify rides through the designated rideshare platform. Monitor transportation services and address delays, cancellations, or changes promptly. Provide professional, compassionate customer service and resolve client concerns. Maintain accurate records of ride requests, bookings, changes, cancellations, and client contacts. Prepare required monthly rideshare reports and verify client and service data. Ensure transportation services comply with program, funding, and confidentiality requirements.

    Qualifications

    Bachelor's degree in Social Work, Psychology, Public Health, Human Services, or a related field required. At least one year of experience in human services, social services, case management, community health, or a related field; relevant internship or field experience may be considered. Experience providing case management or supportive services to people living with HIV preferred. Experience with housing or rental assistance, transportation coordination, or other supportive services preferred. Working knowledge of community resources and social service systems, particularly those addressing housing instability and barriers to healthcare. Knowledge of HIV care systems, Ryan White services, Ending the HIV Epidemic (EHE) initiatives, and community-based support programs preferred. Knowledge of behavioral health, substance use disorders, and related community resources preferred. Ability to work respectfully and compassionately with people living with HIV and diverse populations, including LGBTQ+ communities and individuals experiencing poverty, homelessness, or barriers to healthcare. Strong communication, interpersonal, organizational, documentation, and problem-solving skills. Ability to maintain confidentiality and exercise sound judgment when handling sensitive client information. Ability to collaborate effectively with clients, families, healthcare providers, community agencies, and multidisciplinary teams. Ability to manage multiple priorities, meet deadlines, and work effectively in a fast-paced environment. Ability to respond professionally to behavioral health, substance use, housing instability, and other client challenges. Experience using electronic case management or client information systems preferred. Valid Texas Driver License, automobile insurance, and reliable transportation required.
