    Job Summary
    The Nurse is responsible for addressing the medical needs of assigned individuals in our program. This role includes conducting health assessments and developing and implementing care plans. The Nurse oversees all nursing services delivery, including labor, supplies, pharmaceuticals, durable medical equipment, and billing, while providing direct supervision of participant health evaluations and processes.
     
    Key Responsibilities
    • Monitor and administer medications and provide education to individuals based on their treatment plans.
    • Coordinate all medical care, including appointments with medical, dental, and vision providers.
    • Develop and maintain comprehensive goal plans in collaboration with individuals and their families.
    • Educate and support individuals' families throughout the care process.
    • Ensure compliance with all regulatory requirements and agency policies.
    • Perform other duties as required.
    Qualifications
    • Must be trained on ISPs for all supported individuals.
    • Completion of 24 hours of agency-mandated annual training.
    • Graduate of an accredited school of nursing with an active Registered Nurse license in Pennsylvania. LPN or BSN required; MSN preferred.
    • At least three (3) years of supervisory experience, preferably in an IDD/behavioral health care setting.
    • Demonstrated leadership abilities with strong oral and written communication skills.
