Job Title: Inpatient Pharmacist – Acute Care (Travel Contract)
Location: Santa Rosa, CA 95405
Schedule: Day Shift | 5x8-hour shifts (06:00–14:00 or variable start times including 12:00–20:00)
Hours: 40 hours per week
Duration: 13 Weeks (Temp-to-Perm Opportunity)


Position Overview

A mid-to-large acute care hospital in Northern California is seeking an experienced Inpatient Pharmacist for a 13-week travel assignment within its hospital pharmacy department.

This role supports comprehensive medication management across inpatient units in a high-volume acute care environment. The pharmacist will be responsible for verifying medication orders, ensuring safe and effective pharmacotherapy, and collaborating closely with physicians, nurses, and multidisciplinary clinical teams.

The facility operates with approximately 277 staffed beds and maintains a strong focus on clinical pharmacy integration, medication safety, and evidence-based practice.


Key Responsibilities

  • Review and verify inpatient medication orders for accuracy, safety, and clinical appropriateness
  • Monitor drug therapy regimens and recommend adjustments based on patient condition and laboratory data
  • Provide clinical consultation to physicians and nursing staff regarding medication selection, dosing, and therapy optimization
  • Ensure compliance with hospital formulary guidelines and evidence-based protocols
  • Participate in medication reconciliation during admissions, transfers, and discharges
  • Prepare and dispense medications in accordance with institutional and regulatory standards
  • Support antimicrobial stewardship and therapeutic drug monitoring initiatives
  • Document clinical interventions and pharmacy activities in Epic EMR
  • Collaborate with interdisciplinary teams to improve patient outcomes

Required Qualifications

  • Active California Pharmacist License (required; no pending licenses accepted)
  • Prior experience in inpatient or acute care hospital pharmacy preferred
  • Strong understanding of pharmacology, therapeutics, and hospital medication systems
  • Proficiency with Epic EMR
  • Ability to work early morning or variable daytime shifts (06:00 or 12:00 start times)
  • Strong communication, clinical judgment, and decision-making skills
  • Ability to perform effectively in a fast-paced acute care environment

Preferred Experience

  • Previous hospital or clinical inpatient pharmacy experience
  • Exposure to medication safety programs and clinical pharmacy services
  • Experience working within multidisciplinary hospital teams
  • Familiarity with high-volume acute care workflows
