Scope of Work

Rehab Coding Audit and education to coders.

Deliverables:

Conduct a review/audit of 5-15 closed inpatient rehabilitation records selected by HRC to include: Medicare only.

Based on IRF PPS and ICD-10 Guidelines and Conventions, need resource to validate documentation supporting, but not limited to, the key elements below:

Impairment Group Code

Etiologic Diagnosis

Tiers

Principal Diagnosis

60% Compliance

Specificity in code selection affecting payment and/or compliance.

Audit should include the following feedback:

Proposed coding changes with rationale.

Potential underpayment and overpayment with rationale

Recommendations for improving current coding practices.

Physician Documentation improvements.
