    What You'll Do

    • Be the first trusted connection for patients and families entering hospice care
    • Lead the admissions process with confidence, compassion, and professionalism
    • Perform in-depth assessments and create individualized plans of care
    • Educate and empower families so they feel supported, informed, and at ease
    • Partner with physicians and interdisciplinary teams to ensure seamless care
    • Provide real emotional support-not just clinical care
    • Coordinate services and guide patients through every step of their journey
    • Mentor and supervise support staff to maintain exceptional care standards
    • Keep accurate documentation while managing your day with independence

    • Current Registered Nurse (RN) license in the State of Texas
    • At least 1 year of nursing experience (hospice or home health a plus)
    • Excellent communication and interpersonal skills
    • Strong organizational and critical thinking abilities
    • Ability to work independently while thriving in a team environment
    • Willingness to travel and take after-hours calls
    • Ability to lift up to 55 pounds and perform physical job duties
    • Commitment to compassionate, patient-centered care
