    Your Impact

    As the Home Health Intake Coordinator, you will perform patient intake, referral coordination, and admissions processes. You will serve as the first point of contact for patients and referral partners, ensuring timely admissions, verifying insurance, coordinating care services, and delivering a seamless transition into home health. You will:

    • Oversee all daily patient intake and referral operations
    • Manage the full intake process, including insurance verification and authorization
    • Coordinate and manage patient scheduling to ensure timely admissions, evaluations, and home health visits
    • Optimize clinician schedules while balancing patient needs, geographic considerations, and staffing resources
    • Serve as a key point of contact for patients, families, physicians, and referral sources
    • Receive, process, and track incoming referrals to support efficient patient onboarding
    • Communicate with clinical and administrative teams to facilitate smooth transitions into care
    • Monitor scheduling changes and proactively resolve conflicts, delays, or coverage concerns
    • Track and report referral data, trends, and performance metrics
    • Identify opportunities to improve services and patient access to care

    Our Requirements

    • High school diploma or equivalent required
    • Minimum of one (1) year of experience in healthcare scheduling, intake, medical office administration preferred
    • Home Health experience a plus
    • Excellent communication, customer service, and relationship-building skills
    • Knowledge of healthcare regulations and business operations
    • Current CPR certification
