We are seeking an experienced, detail-oriented Home Health Intake Coordinator to join our growing healthcare team. In this role, you will serve as the primary point of contact for all incoming patient referrals. You will be responsible for managing the referral-to-admission pipeline, verifying insurance benefits, and coordinating with physicians, discharge planners, and our clinical team to ensure a seamless transition to home health care.
The ideal candidate is a master of multitasking who already knows the ins and outs of home health regulations and thrives in a fast-paced electronic environment.
Key Responsibilities
- Receive, review, and process all incoming home health referrals from hospitals, physicians, and community partners.
- Verify patient insurance eligibility, obtain initial authorizations, and document benefit details accurately.
- Review clinical documentation to ensure patients meet federal, state, and insurance criteria for home health services.
- Utilize Homecare Homebase (HCHB) to input patient data, manage workflows, and track referral status.
- Coordinate with the clinical scheduling team to ensure timely initial nursing/therapy assessments.
- Maintain strong, professional communication with referral sources, families, and internal staff.
Position Requirements
To be considered for this role, you must possess the following qualifications:
- Home Health Experience: Minimum of 2 years of direct experience working within a Medicare-certified home health agency.
- Intake Experience: Minimum of 1 year of dedicated experience in home health intake, referral processing, or insurance verification.
- Software Proficiency: Minimum of 1 year of hands-on experience using Homecare Homebase (HCHB). This requirement is firm.
