Job Title: Integrated Care Nurse Practitioner Location: must live within 20 miles of Tucson, AZ (prefer south side) Salary: $120,000 - $150,000 per year
Job Overview: The Advanced Practice Provider delivers personalized, value-based medical care to patients living with chronic kidney disease and end-stage renal disease. This is a field-based role providing in-home and virtual comprehensive assessments, care coordination, and preventative management. The APP works closely with field nurses, community health workers, and physicians to improve outcomes, reduce avoidable hospitalizations, and enhance quality of life.
This is not an office-based position. The provider must live within 20 miles of Tucson, AZ and be comfortable traveling within their assigned territory.
Key Responsibilities:
Conduct comprehensive annual wellness exams in the home and virtual settings
Perform health assessments and develop individualized treatment plans
Prescribe medications and order diagnostic testing as appropriate
Educate patients and families on disease management and preventive care
Coordinate care with primary care providers, specialists, and ancillary services
Complete documentation accurately and timely, including full history, assessment, and care plan
Identify and close gaps in care through preventive monitoring and collaboration
Participate in integrated care team meetings
Deliver evidence-based care that improves quality metrics and patient satisfaction
Assist patients with accessing educational and support resources
Required Qualifications:
Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Associate license
Board certification (NP: ANCC or AANP; PA: NCCPA)
Current and unrestricted DEA certificate
Minimum of 2 years of direct patient care experience
Valid driver's license, reliable transportation, and car insurance
Ability to practice autonomously without direct supervision
Proficiency with electronic medical records and point-of-care technology
Strong knowledge of HIPAA, OSHA, and applicable federal and state regulations
Preferred Qualifications:
Managed Care, IPA, or Health Plan experience
Experience conducting annual wellness visits in-home or virtually
Bilingual in Spanish preferred
Experience in chronic kidney disease or value-based care environments
