Job Description

NorthBay Integrative Primary Care (IPC) believes that lasting health is created through sustainable lifestyle change, meaningful patient partnerships, and whole-person care. The Integrative Primary Care RN serves as a trusted partner within the Integrative Primary Care Program, working in close collaboration with the Program MD's and an on-site interdisciplinary team - including RNs, dietitians, and movement, stress management, and sleep specialists, and medical acupuncturists - to offer a highly personalized, relationship-based care experience built around a smaller patient panel. The Integrative Primary Care Program measures success not only by the clinical care delivered, but by patients' ability to feel informed, supported, and engaged as active partners in their own health. Within the integrative care model, the RN supports patients through individual coaching, shared medical appointments, group education, and coordination with the on-site interdisciplinary lifestyle and integrative medicine team, delivering an enhanced, concierge-like care experience. Working collaboratively with patients, providers, and the lifestyle medicine team, the RN serves as the direct liaison between the provider and patient, ensuring questions and concerns are addressed, directing annual lab and on site group learning engagement, coordinating access to NorthBay's network of specialists, and empowering patient participants to achieve meaningful and sustainable lifestyle transformation.
