LCSW – Regional Intake Specialist

Compensation: $80,000–$95,000 OTE/year

Location: Regional Territory (Boynton Beach, FL) — travel to 8–10 post-acute/long-term care facilities

The Role

We're hiring a Licensed Clinical Social Worker to lead behavioral health intakes and care coordination across a regional network of 8–10 post-acute and long-term care facilities. You'll conduct assessments, evaluate patients for behavioral health and Value-Based Care programs, and support the clinical development of Behavioral Health Coordinators in the field.

What You'll Do

• Conduct psychiatric intakes and psychosocial assessments for

  diagnosisand treatment planning.

• Evaluate patients for eligibility across behavioral health and Value-Based Care programs; obtain informed consent.

• Educate patients, families, and facility staff on available behavioral health services.

• Coordinate referrals, transitions of care, and handoffs across the care

  continuum.
• Serve as clinical escalation point for complex cases and conflict resolution.
• Maintain compliant EHR documentation and review Coordinator documentation for accuracy.
• Mentor and train Behavioral Health Coordinators, including onboarding new team members.
• Flag operational and staffing barriers to leadership.

• Support audits, quality reviews, and compliance initiatives.

• Travel across a defined region to support 8–10 facility sites.

What You Bring

• Active LCSW license (or equivalent independent clinical licensure) in good standing.
• Experience with psychiatric intakes, psychosocial assessments, or behavioral health case management.

• Comfort with EHR systems and digital communication tools.

• Experience with post-acute, long-term care, or geriatric populations a plus.

• Reliable transportation and willingness to travel regionally.
