Social Worker Position Description:
PRHC is seeking an experienced and compassionate licensed Social Worker to provide a range of individualized services to patients with significant medical and cognitive disabilities, and to provide guidance for their caregivers. We are offering a 6-month contract, going month-to-month thereafter. The Social Worker will report to our Nursing Administrator as an integral member of the Social Services Department. This full-time position will be on-site with a Monday through Friday 8:00 – 4:30 schedule, weekends and holidays off.
This position is responsible for psychosocial assessments, identifying health-related social needs for each family on the assigned caseload. They are responsible for planning and implementation of the patient's treatment and discharge plan, which reflects the patient's social work needs from admission to discharge, and which requires added collaboration and coordination responsibilities.

The Social Worker role will include the following scope of work:
  • Primary social worker for an assigned caseload of medically complex pediatric patients.
    • Completes timely and accurate psychosocial assessments according to The Joint Commission (TJC) and Pappas Rehabilitation Hospital for Children (PRHC) standards.
    • Builds trusting relationships with patients and families to improve communication and better understand their needs.
    • Participates in the admissions process, beginning with pre-admission evaluations through the day of admission activities.
    • Provides ongoing support and consultation to caregivers, particularly as it relates to health-related social needs.
    • Develops and coordinates discharge plans in conjunction with community providers and other state agencies, considering the patient's medical, developmental, cultural, ethnic, gender, sexual orientation, and gender identity issues that impact the patient's community needs.
    • Supports patient discharge activities, beginning on the day of admission, through caregiver education on the transition process, identification of patient discharge options, and coordination with community providers to ensure continuity of care.
    • Oversees guardianship process for patients when indicated.
    • Continuously evaluates patient/family functioning and progress
    • Serves as the primary liaison to the Department of Children and Families (DCF) for patients in DCF custody on assigned caseload.
    • Actively participates in Multi-disciplinary Rounds, Patient Care Conferences, group supervision, and other meetings as indicated.
    • Facilities Transition and Family Team Meetings.
    • Participates in PRHC Committees, data collection, and SW performance improvement activities.
    • Completes timely and accurate documentation in the electronic health record according to TJC and PRHC standards.
    • May be required to travel to bring patients to appointments and to other meetings and resources in the community.
    • Perform other assigned duties.
Education and Licensure Requirements:
  • Bachelor's degree or higher degree in social work, psychology, sociology, counseling, counseling education, criminal justice or a relevant human services degree
AND
  • A current and valid licensure as a Licensed Social Work Associate, Licensed Social Worker, Licensed Certified Social Worker, or Licensed Independent Clinical Social Worker issued by the Massachusetts Board of Registration


Qualifications Required at Hire:
  • Knowledge of principles and methods of social casework, group work and community organizations.
  • Ability to make decisions, act quickly and maintain a calm manner in a stressful and/or emergency situations.
  • Skilled in the capacity to exercise professional judgement in "at risk” situations (i.e.. child abuse, suicidal tendencies)
  • Competence in exercising discretion in handling confidential information.
  • Capacity to interact with individuals who are under physical and/or emotional stress and stressful situations.
Preferred Qualifications:
  • Experience with youth and young adults with physical disabilities.
  • Clinical competencies in the delivery of services to youth and families.
  • Knowledge of psychosocial assessments and treatment planning.
  • Knowledge and skills in examining cultural factors at work with individuals and families
  • Familiarity with treatment/intervention programs for school-aged, adolescent, and young adult populations with physical disabilities.
  • Highly organized and able to complete multiple tasks in a fast-paced environment.
  • An appreciation of the importance of a multidisciplinary team, and what patient care departments bring to the patient care endeavor, and the strength of that partnership.
  • Strong written and oral communication skills, with proficiency in Microsoft Office applications including Excel, Outlook, Teams and Word.
