Lead Case Manager (Marengo)

Position: Case Manager lead Program: Homeless Services Position reports to: Homeless Services Manager Status: Full time/Non- Union

Position Summary: The Homeless Services Case Manager is a key member of the multidisciplinary team dedicated to supporting the homeless population. This combined role focuses on providing outreach, case management, resource navigation, and coordination of healthcare and housing services to individuals experiencing homelessness. The position involves working both in-house and in the field collaborating with social services professionals, healthcare providers, and community organizations to ensure access to essential resources such as food, shelter, medical care, housing, and other supportive services.

Key Responsibilities:

Outreach & Engagement: Actively engage with individuals experiencing homelessness in both street-based and walk-in settings, providing outreach and building trust. Assist with connecting them to healthcare, housing, and community resources, and address barriers to care. Case Management & Care Coordination: Develop and implement individualized care plans, offering ongoing case management, follow-up, and advocacy to ensure access to services. Ensure clients are linked to appropriate housing, food, healthcare, and social services. Assist clients in completing necessary paperwork to enroll in services, including healthcare, public assistance, and housing programs. Crisis Intervention Expertise: Take on a leadership role in complex cases, offering specialized crisis intervention, de-escalation, and conflict resolution.Provide expert guidance to less experienced staff on how to handle high-risk or challenging situations. Resource Development: Lead the development of new client resources, ensuring that services are continually adapted to meet evolving needs.Take the initiative to bring new resources or partnerships into the program that increase the capacity to support clients, including new housing opportunities or healthcare access. Collaboration: Collaborate with Clinica Romero programs and providers like Enhanced Care Management (ECM), Behavioral Health, Substance Use Department, and Referrals. Housing Assistance & Navigation: Link clients to bridge/interim housing programs and assist with applications for permanent housing-related programs. Guide clients through the housing process and connect them with local shelters and other housing resources. Healthcare & Supportive Services: Assist clients in accessing healthcare services, including primary care, mental health services, substance use treatment, and dental care. Help clients obtain identification, enroll in health insurance, and access transportation to services. EMR Documentation & Data Entry: Utilize Electronic Medical Records (EMR) systems (e.g., EPIC) to maintain accurate and confidential patient records. Enter appropriate data into Homeless Management Information Systems (HMIS) and other required databases. Monitoring & Reporting: Perform daily follow-ups, wellness checks, and case updates to ensure clients' needs are consistently met. Maintain organized files and reports for each client. Training: Providing case management training to new staff hired, including Community Care Coordinators and support staff.

Qualifications/Requirements:

Bachelor's degree in social work, human services, public health, nursing, or a related field, or equivalent experience. Strong knowledge of local housing resources and services, particularly in Los Angeles. Experience in case management, resource navigation, and using EMR systems (e.g., EPIC) is preferred. Proficiency in MS Office Suite and ability to manage multiple cases effectively. Bilingual (English/Spanish) is a plus. Strong communication and interpersonal skills with the ability to build rapport with diverse populations, particularly vulnerable individuals. Highly organized and able to work in a fast-paced environment, managing multiple tasks while maintaining attention to detail. Effective crisis management and problem-solving skills. Ability to maintain client confidentiality and adhere to safety and workplace policies.
