Licensed Independent Clinical Social Worker

Perm candidates of any OTHER providence facility ( NON SWEDISH ) are eligible for a travel contract if they have been separated for at least 3 months. All Swedish facilities honor active compact license.

Position: Social Worker for Pre/Post Liver Transplant Patients
Schedule: 5x8 Days Mon-Friday
Weekends: No

Case Load varies from 10 new patients a week
verifying caregivers over the phone

*completing psychosocial evaluations for Pre/post liver transplant patients

stationed outpatient, but do provide evaluations for inpatient, depending on dept need
