Title: Licensed Master Social Worker / Geriatric Care Manager (home visits required)
Location: Queens, Richmond Hill, NY (Onsite)
Position Type: Full Time - Mon-Fri (Field visits throughout the service area - Manhattan, Brooklyn and Queens)
Compensation
Pay Range:$70,000-$75,000 Per Year
Description
We are looking for a compassionate Geriatric Care Manager/ Licensed Master Social Worker specializing in geriatric care to join our client's team.
This full-time position is based in New York, NY, with a Monday to Friday schedule.
The role involves working in the field and collaborating with a supportive team, requiring proficiency in general office software and electronic documentation systems.
Requirements
Conduct regular home visits in Manhattan, Brooklyn and Queens.
Possess a valid driver's license and reliable transportation.
Attend weekly in-person team meetings at our office.
Maintain flexibility to adjust schedules based on client needs.
Be comfortable working independently and as part of a team.
Strong organizational and time-management skills.
Excellent interpersonal and communication skills.
Qualifications
LMSW or LCSW required.
Experience in working with older adults and care management.
Knowledge of Medicare, Medicaid, Social Security, and community services.
Proficiency with general office software and electronic documentation systems.
