Are you a mental health professional with a passion for making a positive impact on the lives of caregivers for older adults experiencing neurocognitive decline? If so, we have the perfect opportunity for you to lead our caregiver and clinical services program based in Springfield, Vermont. 
 
At Senior Solutions (Area Agency on Aging in Southeastern Vermont), we are seeking a Caregiver and Clinical Services Coordinator to work with caregivers of older adults experiencing neurocognitive decline throughout our service area in southeastern Vermont. 
 
This position provides comprehensive person-centered, services and program support for Vermonters caring for loved ones with Alzheimer’s Disease, related dementias or other neurocognitive disorders. This position plays a key role in helping older adults remain safely and independently in their homes, by supporting family caregivers in managing the emotional, physical, and social demands of caregiving. The position also supports our complex care case management team with clinical oversight.
 
Qualifications:
Licensed Mental Health Counselor with at least three years of social service or related experience working for a human services organization that provides assessment and direct services to older adults.

Experience working with older adults and younger disabled adults in a community setting.

Education and experience in working with individuals living with Alzheimer's Disease, related dementias and/or neurocognitive disorders.

Strong administrative skills including timely and accurate reporting of data and information.

Experience with Medicaid and Medicare billing procedures.

Strong proficiency in use of information technology, computerized databases, and computer software, including Windows 11.

Physical Demands/Working Conditions

Travel to visit with clients and to represent the agency at community partner meetings.

Must have a valid driver's license and access to reliable transportation.

Ability to occasionally lift and/or carry objects weighing up to 25 pounds.

Must possess the physical capabilities to perform the following: reaching, handling, talking, hearing, seeing, standing, walking, stooping, and bending.

Work environment involves minimal exposure to physical risks.

Recordkeeping work is normally performed in a typical interior/office work environment.
