KEY RESPONSIBILITIES:
Assist CNAs with ADLs and routine resident care
Administer medications and treatments as prescribed
Accurately document and maintain patient records
Observe and report resident health conditions to RNs, supervisors, and physicians
Take vital signs and monitor fluid/food intake and output
Provide wound care, injections, and IV management
Support the admissions process and communicate with the pharmacy for orders
Ensure resident safety by reporting incidents, accidents, or skin breakdowns
QUALIFICATIONS:
Current LPN license in the state of New Jersey
Strong clinical and communication skills
Compassionate, reliable, and team-oriented
