Job Purpose:
The Licensed Practical Nurse at Crescent Community Health Center is responsible for providing high-quality, patient-centered healthcare services within the community. This role involves collaborating with healthcare teams to deliver compassionate care, ensuring patient comfort, and promoting overall health and wellness.
Key Responsibilities:
Administer medications and treatments as prescribed by physicians and in accordance with nursing standards and protocols.
Monitor patient health by conducting regular assessments, recording vital signs, and observing any changes in condition.
Assist in the development and implementation of individualized patient care plans, ensuring they meet the specific needs of each patient.
Provide education and support to patients and their families regarding health maintenance, disease prevention, and treatment options.
Collaborate with physicians, registered nurses, and other healthcare professionals to ensure cohesive and comprehensive patient care.
Maintain accurate and up-to-date patient records, documenting all care provided and any changes in patient condition.
Participate in quality improvement initiatives to enhance patient care and operational efficiency within the health center.
Ensure compliance with all healthcare regulations, policies, and procedures to uphold the highest standards of practice.
Assist in maintaining a clean, safe, and organized healthcare environment for patients and staff.
Engage in continuous professional development to stay current with nursing practices and healthcare advancements.
Required Education:
Completion of an accredited Licensed Practical Nurse (LPN) program.
Current LPN license in the state of practice.
Required Experience:
Minimum of 1-2 years of experience working as a Licensed Practical Nurse in a healthcare setting.
Experience in providing direct patient care and managing patient records.
Required Skills and Abilities:

Excellent communication skills to interact effectively with patients, families, and healthcare team members.
