Position Overview:
Seeking an experienced Licensed Practical Nurse to provide high quality, patient-centered care in a home health setting. This role supports the interdisciplinary care team by delivering skilled nursing services in accordance with physician orders, agency policies, and state regulations. The ideal candidate has prior home health experience and is comfortable working independently in the field.

Schedule & Work Details:

  • Location: Field-based role serving the Eureka Springs area
  • Schedule: 8:00 AM to 4:30 PM
  • Weekly Hour Guarantee: 32 hours per week
  • On-Call Requirements: On call approximately 1 time per month, with participation in weekend rotations and potential holiday coverage
  • Mileage Reimbursement: Yes, reimbursed between patient visits
  • Pay Rate: $28 to $30 per hour

Key Responsibilities:

  • Provide high quality clinical services within scope of practice, following state nursing laws, physician orders, and the established plan of care
  • Deliver patient care in coordination with the RN and interdisciplinary team without altering the plan of care
  • Observe, assess, and report patient condition and nursing needs to the supervising RN
  • Communicate effectively with physicians, patients, families, and agency staff regarding patient status, care needs, scheduling, and visit updates
  • Schedule and complete home visits as assigned and coordinate care with other clinicians
  • Accurately complete and submit clinical documentation, including visit notes, OASIS-related data, and medication lists in compliance with agency and payer requirements
  • Maintain timely documentation and system synchronization per company policy
  • Participate in performance improvement initiatives focused on reducing hospitalizations and enhancing patient outcomes
  • Attend and actively participate in meetings, including case conferences, and maintain punctuality and professionalism
  • Demonstrate a positive attitude, adaptability, and willingness to learn and support team needs
  • Maintain schedule flexibility to meet patient care demands, including on call, weekends, and holidays as needed
  • Participate in the agency’s Episode Management process
  • Perform additional duties as assigned

Required Qualifications:

  • Licensed Practical Nurse (LPN) with an active and unrestricted license in the applicable state
  • Prior home health experience required
  • Strong understanding of clinical care processes, documentation standards, and patient safety practices
  • Ability to work independently and manage time effectively in a field setting
  • Reliable transportation and ability to travel within the service area

Preferred Qualifications:

  • Experience with Homecare Homebase (HCHB) EMR system
  • Strong communication and organizational skills
  • Ability to collaborate effectively within a care team environment

Required Certifications:

  • Current LPN license
  • Current CPR certification
