A.O. Fox Nursing Home is a 130-bed hospital-based long-term care facility. We offer post-acute rehabilitation to get community members back home and make a welcoming home for those requiring 24-hour residential skilled nursing care. We take the time to train new employees well as we embrace the “Fox Family” culture, providing a supportive and safe environment, with good staffing and a caring environment. Responsibilities:
Under the direction of the Registered Nurse, the Licensed Practical Nurse II provides nursing care in accordance with departmental and hospital policies and procedures, supporting the care plan established for each patient.
- Transfer, reposition, ambulate patients 1-2 hours or PRN, collects samples and monitors catheters.
- Identifies patient care problems and communicates them to the registered nurse.
- Maintains up to date and accurate documentation of nursing care in the EMR.
- Administers medications and immunizations as ordered.
- Performs and assists with procedures as needed ensuring all supplies are available and set up according to protocol.
- Practices within the scope of practice 100% of time.
- Assumes self-responsibility for direct care activities as indicated by the RN.
- Gives direct patient care according to the therapeutic plan and individual patient needs as directed by the patient care plan.
- Provides appropriate information to assist the RN ion the preparation and Maintenace of an effective care plan.
- Records patients vital signs, ADL, safety checks in the EMR.
- Notifies the RN of significant findings or changes in the patient's physical or mental conditions.
- Acts as a role model for ancillary staff.
- Insures privacy for patient and confidential of condition and personal an information.
- Maintains the confidentiality of the patients clinical records.
- Overseas CNA's to ensure proper care is provided and unit flows appropriately.
- Maintains LPN NYS requirements.
- BLS Certification when appropriate.
- Intravenous medication administration after RN has given the first dose.
Qualifications:
Education:
- Completion of an Licensed Practical Nurse program, required
- LPN II Course, required
Experience:
- Previous long term care experience, preferred
License/ Certification:
- Current NYS Licensure as a Practical Nurse, required
- BLS certification within 6 months of hire, required
