Position Overview

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) for a part-time weekend home care opportunity in Blandon, PA.

This position involves providing individualized care and support to a client with intellectual and developmental disabilities (IDD) in a private home setting.

The client receives 2:1 care, meaning two caregivers are assigned to provide support. The ideal candidate is comfortable assisting with activities of daily living (ADLs), mobility, transfers, and general nursing care within their licensed scope of practice.

Key Responsibilities

• Assist with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and personal hygiene.
• Provide safe transfers, repositioning, and mobility assistance.
• Monitor the client's overall health, comfort, and well-being.
• Provide nursing care and medication administration as directed by the client's care plan and within LPN scope of practice.
• Work collaboratively with the second assigned caregiver to ensure safe, consistent care.
• Maintain accurate care documentation and communicate changes in the client's condition.
• Promote dignity, independence, and quality of life.

Qualifications

• Active, unrestricted Pennsylvania LPN license.
• Previous home care, private duty nursing, or IDD experience preferred.
• Experience assisting with ADLs, transfers, and mobility support.
• Current CPR/BLS certification preferred.
• Dependable transportation to Blandon, PA.
• Ability to consistently work both Saturday and Sunday, 8 AM 8 PM.
