Job Title: Licensed Practical Nurse (LPN)- ElderONE North Park (Part-Time, Days or Evenings)

Location:  2066 Hudson Avenue, Rochester, NY 14617

Hours Per Week: 20

Schedule: Day and Evening Shifts: (Days 7:00am to 3:30pm, Evenings 3:00pm to 11:30pm), every other weekend required.

SUMMARY:The Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans.RESPONSIBILITIES:

• Conducts and documents a thorough observation of each patient’s medical status upon admission and throughout the patient’s course of treatment.
• Assists in the implementation of the patient’s treatment plan under the direction of the RN.
• Provides routine nursing services for patients as directed by the RN.
• Complies with the evaluation, treatment, and documentation guidelines of the department.
• Completes required documentation in an accurate and timely manner.
• Provides regular patient status updates to the appropriate personnel.
• Attends and participates in department/facility meetings as required.
• Acts in compliance with hospital, regulatory, and professional standards and policies.
• Maintains clinical competency by pursuing continuing education opportunities.
• Performs other duties as assigned.

REQUIRED LICENSURE / CERTIFICATION SKILLS

• Graduate from an accredited School of Practical Nursing.   

• Current LPN licensure in State of New York.   

• Basic Life Support (BLS) Certification from the American Heart Association (AHA).  

PREFERRED QUALIFICATIONS

• 1 year experience

• 1 year of experience working with a frail or elderly population preferred.
