Role Summary

The Licensed Practical Nurse (LPN/LVN) plays a vital role in delivering high-quality, compassionate primary care services within a clinic setting serving a frail and elderly population. Working under the direction of the Clinic Practice Manager and in close collaboration with physicians, Registered Nurses, and an interdisciplinary care team, the LPN/LVN supports comprehensive, patient-centered care through medication administration, clinical assessments, participant education, and care coordination. This role offers the opportunity to make a meaningful impact while working in a collaborative, mission-driven healthcare environment.


Key Duties & Responsibilities

  • Perform delegated nursing services within the clinic setting in accordance with scope of practice and under the supervision of the Clinic Practice Manager

  • Collaborate with the Interdisciplinary Team and RN Care Manager to plan and implement interventions aligned with each patient’s individualized plan of care

  • Administer prescribed medications, vaccinations, and treatments following nursing standards and document all care appropriately

  • Withdraw blood from patients as instructed and within scope of practice, as designated

  • Maintain accurate and timely medical records and meet all charting and reporting requirements

  • Observe, document, and report patient conditions and responses to medications or treatments to physicians and care team members

  • Triage patients within scope of practice to determine urgency and priority of care

  • Educate patients and family members/caregivers on medications, treatments, and care instructions

  • Prepare equipment and assist providers during examinations, treatments, and diagnostic testing

  • Support clinic operations by assisting with daily appointments, patient flow, and emergency situations

  • Respond to incoming telephone calls and appropriately route or address concerns

  • Maintain exam rooms, ensure cleanliness, stock necessary supplies, and place items on reorder lists as needed

  • Monitor expiration dates of medications and clinic supplies on a scheduled basis

  • Communicate effectively during interdisciplinary team meetings, clinic meetings, and other assigned meetings

  • Assist, as assigned, with scheduling specialist appointments, coordinating hospital admissions, and providing reports to receiving units

  • Follow all organizational policies, procedures, and OSHA safety guidelines

  • Maintain confidentiality of all patient, family, and organizational information

  • Handle potentially infectious specimens using appropriate biohazard and universal precautions

  • Maintain a safe work environment and adhere to established safety protocols

  • Participate in quality improvement initiatives and performance enhancement activities

  • Engage in required continuing education, staff training, and professional development

  • Perform additional duties as assigned to support clinic and patient care operations


Required Qualifications

  • Graduate of an accredited school of nursing

  • Current LPN/LVN license in good standing in the required state

  • Basic Life Support (BLS) certification (must be obtained within 90 days of employment)

  • Reliable transportation, valid driver’s license, and minimum state-required auto liability insurance

  • Medical clearance for communicable diseases with immunizations up to date prior to direct patient contact

  • Ability to pass a comprehensive pre-employment background check, including license verification, education verification, reference checks, exclusion lists, and drug screening


Desired Qualifications

  • One year of experience working as an LPN/LVN (strongly preferred)

  • IV Certification (preferred)

  • At least one year of experience working with a frail or elderly population (preferred)

    • If not present, training will be provided upon hire

  • Strong communication, teamwork, and patient-education skills

  • Ability to work effectively in a fast-paced, interdisciplinary clinic environment
