Licensed Practical Nurse (LPN)

• Provides skilled nursing care in a patient’s home as ordered by the attending physician.

• Performs care by following order sets and goals made by RN.

• Observes and monitors patient conditions.

• Notifies RNCM of any changes in patient condition.

• Administers medicationas prescribed bythe physician.

• Counselspatientand family on the disease/injury processes and how to manage.

• Incorporatespatientand family indevelopmentofplan of care.

• Helps decrease re-hospitalizationsby teaching the patient on preventative measures and making good decisions.

• Listens to patient and family members.

• Completes all clinical documentationin accordance withagency protocol and Medicare/Federal guidelines.

• Documentsall aspects of treatment, assessments, and patient education.

• Maintains active LPNlicensein state of employment.

• Communicates withschedulerany changes outside the normal.

• Participates in all on-call requirements and case conferences.

• Attends in-servicetrainingsand mandatory agency meetings.

• Maintains a professional appearance as a representative of the company.

• Understands and practices agency policies and procedures.

• Drives topatient’sprimary location per scheduled visit.

• Daily attendance at assigned visit locations.

• Documents all aspects ofsubsequentvisits within 24 hours of visit to meet Medicare guidelines. Completes andsubmitsall required documentation within specified company requirements.

• Knowledgeable with providing skilled care within the scope of practice for a Licensed Practical Nurse.

• Fulfills LPN visits in assigned geographic location per patientneedwithin federal and state specifications.

• Other duties as assigned.

• Minimumof one year of experience asaLPN in an acute care setting is; two years is preferred. Home care experience is desired.

• Ability to effectively communicate and create positive impressions with patients, families,physiciansand co-workers.

• Ability to remain calm, havepatienceand beaccommodating. Compassionate and caring while working with patients.

• Knowledgeable onnursing best practices.

• Ability to makeappropriate nursingjudgments.

• Ability toidentifya situation and handle it with the best possible solution.

• Disciplined style of work ethic with the ability to prioritize and betimely

• Ability to follow directions and work as a team member.

• Valid driver’s license and auto insurance with your name as a listed driver.
