Position Title: Licensed Practical Nurse (LPN) Location: Falmouth, Massachusetts 02540 Weekly Hours: 40 Hours Schedule: Evening Shift | 3:00 PM – 11:30 PM | Every Other Weekend | Rotating Holidays Duration: 13 Weeks (Strong Possibility of Extension or Permanent Placement)
Position Overview
"Navitas Healthcare, LLC" is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join a long-term care team in Falmouth, Massachusetts. The LPN will provide direct nursing care to residents, supervise support staff, and help ensure high-quality patient outcomes in compliance with healthcare regulations and facility standards.
Key Responsibilities
Provide direct patient care and administer medications as prescribed
Supervise and support nursing assistants and other care staff
Maintain accurate documentation for admissions, transfers, and discharges
Implement physician orders and individualized care plans
Monitor patient conditions and document treatment responses
Perform clinical procedures including wound care, catheterization, tube feeding, and specimen collection
Collaborate with physicians and interdisciplinary healthcare teams
Ensure proper availability and management of medical supplies and medications
Educate residents and families regarding healthcare needs and treatment plans
Participate in audits, inspections, and compliance-related activities
Required Qualifications
Strong understanding of nursing practices, patient care procedures, and healthcare regulations
Ability to supervise staff and effectively manage resident care
Excellent communication and interpersonal skills
Ability to work independently and make sound clinical decisions
Education
Graduate of an accredited nursing program or approved Licensed Practical Nursing (LPN) program
Licensure & Certifications
Active and unrestricted Massachusetts LPN License
Current BLS / CPR / AED Certification
Preferred Experience
Prior experience in long-term care, rehabilitation, or skilled nursing settings preferred
Strong documentation and charting skills
Ability to communicate effectively in English, both written and verbal
Ability to meet physical requirements, including lifting up to 50 lbs
