Position Title: Licensed Practical Nurse (LPN)
Location: Falmouth, Massachusetts 02540
Weekly Hours: 40 Hours
Schedule: Evening Shift | 3:00 PM – 11:30 PM | Every Other Weekend | Rotating Holidays
Duration: 13 Weeks (Strong Possibility of Extension or Permanent Placement)

Position Overview

"Navitas Healthcare, LLC" is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join a long-term care team in Falmouth, Massachusetts. The LPN will provide direct nursing care to residents, supervise support staff, and help ensure high-quality patient outcomes in compliance with healthcare regulations and facility standards.

Key Responsibilities

  • Provide direct patient care and administer medications as prescribed
  • Supervise and support nursing assistants and other care staff
  • Maintain accurate documentation for admissions, transfers, and discharges
  • Implement physician orders and individualized care plans
  • Monitor patient conditions and document treatment responses
  • Perform clinical procedures including wound care, catheterization, tube feeding, and specimen collection
  • Collaborate with physicians and interdisciplinary healthcare teams
  • Ensure proper availability and management of medical supplies and medications
  • Educate residents and families regarding healthcare needs and treatment plans
  • Participate in audits, inspections, and compliance-related activities

Required Qualifications

  • Strong understanding of nursing practices, patient care procedures, and healthcare regulations
  • Ability to supervise staff and effectively manage resident care
  • Excellent communication and interpersonal skills
  • Ability to work independently and make sound clinical decisions

Education

  • Graduate of an accredited nursing program or approved Licensed Practical Nursing (LPN) program

Licensure & Certifications

  • Active and unrestricted Massachusetts LPN License
  • Current BLS / CPR / AED Certification

Preferred Experience

  • Prior experience in long-term care, rehabilitation, or skilled nursing settings preferred
  • Strong documentation and charting skills
  • Ability to communicate effectively in English, both written and verbal
  • Ability to meet physical requirements, including lifting up to 50 lbs
