Position Summary
"Navitas Healthcare, LLC" is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality nursing care in a skilled nursing and long-term care setting. This role focuses on delivering safe, patient-centered care during overnight shifts, including medication administration, clinical monitoring, and coordination with the interdisciplinary care team.
Key Responsibilities
- Administer medications and treatments as prescribed by physicians
- Monitor residents’ conditions and promptly report any changes
- Perform routine nursing assessments and accurately document care provided
- Provide wound care and other basic nursing procedures within scope of practice
- Maintain complete and accurate clinical documentation and charting
- Assist with admissions, discharges, and transfers as needed
- Support Certified Nursing Assistants (CNAs) in resident care activities when required
- Ensure adherence to infection control and safety protocols at all times
- Respond to emergencies and resident needs during night shifts
- Collaborate effectively with the interdisciplinary healthcare team
Required Qualifications
- Active and unrestricted LPN license (Colorado or compact state eligibility)
- Prior experience in long-term care or skilled nursing preferred
- Ability to work consistent overnight 12-hour shifts
- Strong clinical judgment and documentation skills
- Ability to work independently and manage patient care during night shifts
