Job Summary:
We are seeking a dedicated Licensed Practical Nurse to provide high-quality care in a long-term care and rehabilitation setting. The role involves medication administration, patient monitoring, and supporting residents with chronic and cognitive conditions in a structured care environment.

Key Responsibilities:

• Administer medications and perform medication pass (Med Pass) duties
• Provide direct patient care in a long-term care setting
• Monitor and support residents with Alzheimer’s and dementia
• Assist with daily living activities and patient safety
• Document care accurately using facility systems
• Collaborate with nursing staff to ensure quality resident care

Facility Overview:

• Approximately 300-bed long-term care and rehabilitation center
• Patient population includes Alzheimer’s and dementia care residents

Requirements:

• Active New York State LPN/LVN license required (must be in hand at submission)
• Minimum 1 year of nursing experience required
• Long-term care experience preferred (rehab, med-surg, or hospital experience also accepted)
• BLS certification required
• COVID-19 vaccination required
• First-time travelers accepted
• Must be willing to work every other weekend
