The LPN will provide high-quality nursing care to our patients in a home care setting, ensuring their comfort, safety, and well-being. Responsibilities

• Provide skilled nursing care to patients in their homes, following care plans and physician instructions.
• Monitor patients’ conditions and report any changes or concerns to healthcare providers.
• Administer medications and treatments as prescribed.
• Assist patients with activities of daily living as needed.
• Maintain accurate and timely documentation of patient care and progress.
• Collaborate with healthcare team members to coordinate patient care.
• Educate patients and families on health conditions, treatments, and healthcare management.

Requirements

• Valid Licensed Practical Nurse (LPN) license in the state of Michigan.
• Previous experience in home health, clinical, or related nursing settings preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Compassionate and patient-centered approach to care.
• Basic computer skills for documentation purposes.
