Job Description:
Nightingale's List is seeking skilled and compassionate Licensed Practical Nurses (LPNs) to join our team. In this role, you will provide essential patient care across diverse healthcare environments, delivering high-quality support to ensure the health and well-being of all patients.
Key Responsibilities:
Administer medications and treatments as prescribed by physicians.
Monitor patient health by assessing vital signs, performing wound care, and documenting patient conditions.
Assist in the development and implementation of individualized patient care plans.
Provide bedside care, supporting patients with activities of daily living as needed.
Observe and report any changes in patient conditions or concerns to the supervising RN.
Follow infection control protocols to maintain a clean and safe environment.
Support patients with mobility needs, including turning, repositioning, and transferring.
Educate patients and families on healthcare instructions and follow-up care.
Requirements:
Must have an active LPN license in the state of Vermont.
Minimum of 1 year of LPN experience, preferably in correctional, nursing home, or rehabilitation settings.
CPR certification is required; additional certifications are a plus.
Excellent communication skills and ability to collaborate effectively with a healthcare team.
Empathetic, detail-oriented approach to patient care.
