Position Overview

The Licensed Practical Nurse (LPN) is responsible for providing high-quality nursing care to individuals with developmental and intellectual disabilities in a home and group home environment. The LPN plays an essential role in monitoring resident health, administering medications, supporting activities of daily living, and ensuring care is delivered in accordance with physician orders and individualized care plans.

This role requires strong clinical awareness, compassion, teamwork, and the ability to provide hands-on care while maintaining a safe and supportive environment for residents.

Reports to: Director of Nursing (DON)

Key Responsibilities

Direct Nursing Care & Resident Monitoring

  • Provide direct nursing care in accordance with individualized care plans and physician orders
  • Perform focused nursing assessments at the beginning of shifts and as needed
  • Monitor residents for changes in physical, emotional, or behavioral condition and report concerns appropriately
  • Collect, document, and interpret vital signs and other health-related observations
  • Assist in maintaining resident comfort, safety, and overall well-being

Medication Administration

  • Administer medications safely and accurately following the seven rights of medication administration
  • Monitor residents for medication effectiveness and adverse reactions
  • Maintain accurate and timely Medication Administration Records (MARs)
  • Report medication concerns or significant observations to the supervising nurse or physician promptly

Collaboration & Care Coordination

  • Collaborate with RNs, CNAs, physicians, support coordinators, and interdisciplinary team members
  • Communicate changes in resident condition, incidents, or concerns effectively and professionally
  • Participate in care planning meetings and assist with updates to individualized care plans
  • Support continuity of care through strong communication with residents, caregivers, and families

Personal Care & Daily Living Support

  • Assist residents with mobility, transfers, ambulation, and activities of daily living as needed
  • Support proper hygiene, grooming, feeding, and personal care while preserving resident dignity and independence
  • Ensure dietary and physician-directed care instructions are followed appropriately
  • Provide hands-on support and oversight to direct care staff when necessary

Documentation & Compliance

  • Complete nursing documentation accurately and timely within the electronic health record system
  • Maintain confidentiality and compliance with HIPAA and organizational standards
  • Document changes in resident condition, interventions performed, and care provided
  • Participate in quality assurance and compliance initiatives as directed

Safety & Emergency Response

  • Maintain a safe environment for residents and staff by following organizational safety protocols
  • Respond appropriately to emergencies and changes in resident condition
  • Ensure emergency equipment and medical supplies are maintained and accessible
  • Educate residents and caregivers on health, safety, and medication-related practices

Resident Advocacy & Support

  • Advocate for resident rights, dignity, and individualized care needs
  • Provide emotional support and compassionate care to residents and families
  • Encourage resident involvement in care decisions and promote independence whenever possible

Qualifications

  • Active Licensed Practical Nurse (LPN) license in the State of Florida
  • Nursing diploma or associate degree required
  • Minimum of one (1) year of nursing experience preferred, ideally in home health, long-term care, or developmental disability services
  • Strong understanding of nursing practices, medication administration, and care coordination
  • Experience working with medically complex residents preferred
  • Excellent communication, interpersonal, and teamwork skills
  • Ability to travel between assigned locations as needed
  • Flexibility to work weekends, holidays, and during inclement weather when required
  • Ability to lift, move, push, or pull 50 pounds with or without reasonable accommodation
  • Current CPR/First Aid certification required or ability to obtain within designated timeframe
