Licensed Practical Nurse (LPN) Wellness Team

Presley Healthcare, LLC is looking for an experienced LPN to become our wellness nurse in our Acute Care Clinics in Jamestown/Clarkrange and Monterey Medical. This position focuses on preventive measures and visits for Medicare patients along with patient outreach for assistance with medications and social determinants of health.

You will also be responsible for follow up of patients who have recently been hospitalized or in a long term care facility, such as a nursing home.

Must be up to date on licensure and certifications including BLS.

Experience in phlebotomy, IV therapy and quality measures is a plus. Although preferred applicants will have 2+years experience in a clinical setting, recent graduates are welcome to apply.

Position is Monday - Friday day shift. NO ON-CALL. No weekends.

Job Type: Full-time

Pay: $18.00 - $20.00 per hour
