About the Role: Presidential Home Care LLC is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing team in Woodstock, GA. Responsibilities:
Provide direct patient care including wound care, medication administration, and vital sign monitoring
Develop and follow individualized patient care plans in coordination with supervising RNs and physicians
Observe and document patient conditions, reporting any changes to the appropriate healthcare team members
Assist patients with activities of daily living (ADLs) and personal hygiene as needed
Educate patients and family members on care procedures, medications, and health management
Maintain accurate and timely clinical documentation and medical records
Coordinate with home care team members to ensure continuity of care
Requirements:
Active and unrestricted LPN license in the state of Georgia
Previous home health, long-term care, or clinical nursing experience preferred
Strong knowledge of nursing procedures, medication protocols, and patient assessment
Excellent communication and interpersonal skills with patients and families
Ability to work independently and manage time effectively in a home care setting
Valid driver's license and reliable transportation required
CPR/BLS certification preferred
