Job Title: Licensed Practical Nurse

Job Description

This Licensed Practical Nurse role is an integral part of the clinical team, responsible for providing safe, evidence-based care to patients while offering exceptional support and anticipating the needs of both clinicians and patients. The position focuses on delivering high-quality nursing care in a supportive, patient-centered environment with strong teamwork and professional collaboration.

Responsibilities

  • Monitor, document, and communicate patients' vital signs, statistics, and measurements in a timely and accurate manner.
  • Observe patients closely and record any physical or emotional changes, escalating concerns to the appropriate clinical staff when necessary.
  • Prepare examination rooms for patient visits, ensuring that all required supplies, instruments, and equipment are available and functioning properly.
  • Select, prepare, and manage medical supplies and equipment to support efficient and safe patient care.
  • Maintain a patient environment that is comfortable, organized, safe, and hygienic at all times.
  • Maintain accurate and up-to-date records of patient information in electronic medical record (EMR) systems and other documentation tools.
  • Prepare patients for follow-up care after treatment, including providing clear instructions and reinforcing care plans as directed by clinicians.
  • Provide guidance and education to other LPNs when needed, supporting a collaborative and learning-focused nursing team.
  • Serve as a knowledgeable resource on nursing-related issues within the unit or section, contributing to clinical decision-making and best practices.
  • Organize and manage scheduling for patients and necessary medical tests to support smooth clinic and facility operations.
  • Demonstrate consistent attendance and reliability to ensure continuity of care and support for patients and colleagues.

Essential Skills

  • Active Licensed Practical Nurse (LPN) license in the state of New York.
  • At least 2 years of experience working as an LPN in a healthcare setting.
  • Proficiency with electronic medical record (EMR) systems for documenting and retrieving patient information.
  • Strong clinical skills in monitoring vital signs and recognizing changes in patient condition.
  • Ability to maintain accurate and detailed patient records and documentation.
  • Commitment to attendance, reliability, and professional accountability.
  • Ability to work collaboratively within a multidisciplinary healthcare team.
  • Knowledge of evidence-based nursing practices and patient safety standards.

Additional Skills & Qualifications

  • Experience in long-term care, skilled nursing, or similar healthcare environments is beneficial.
  • Strong organizational skills for managing patient scheduling and coordinating medical tests.
  • Effective communication skills for interacting with residents, families, and clinical staff.
  • Ability to provide education and guidance to peers and serve as a nursing resource within the unit.
  • Demonstrated commitment to meaningful engagement with residents and patient-centered care.
  • Interest in working in a setting with high standards of care and strong staff-to-resident ratios.

This role is based in a highly rated healthcare facility in New Rochelle, conveniently located near major highways and public transportation, including a Bee-Line 45 bus stop directly in front of the campus. The facility is recognized with a 5-star rating from CMS and maintains strong staffing levels to support excellent and safe care for residents and patients. The work environment emphasizes patient safety, staff satisfaction, collaboration, and respect, with a professional team culture where colleagues build strong relationships and residents share meaningful life stories. The position offers multiple openings and includes an afternoon-to-evening shift from 3:00 PM to 11:00 PM, with every weekend required. This is a Contract to Hire position based out of New Rochelle, NY.

Pay and Benefits

The pay range for this position is $33.04 - $36.35/hr.
