Position Highlights:

- Support patients with daily health needs and educate on treatments and wellness strategies
- Maintain accurate clinical documentation and patient records
- Follow infection control, safety, and regulatory standards
- Active LPN license in Massachusetts
- Completion of an accredited practical nursing program
- 1-2 years of nursing experience preferred, especially in outpatient, community health, or long-term care
- Knowledge of basic nursing procedures and compassionate patient care
- Strong observation, communication, and teamwork skills
- Ability to follow care plans and work effectively under supervision
