Schedule: Up to 40 hours/week – scheduled within a 7 ‑day work period
Salary Range: $28,571.29 – $36,023.24 – $62,000.00
Starting salary is commensurate with education and relevant experience. Advancement within the pay scale is based on demonstrated performance, increased responsibility, and organizational budget capacity.

The LPN 3 plays a vital role in fulfilling this mission and vision by leading and supporting CRA (Community Residential Alternative) setting, CLS (Community Living Support), and Day Services by providing a person-centered approach to nursing care for people with intellectual and developmental disabilities. The LPN 3 ensures full compliance with applicable local, state, and federal regulations, including those from DBHDD, DCH, and CMS. This position is essential in maintaining a safe and respectful environment that reflects each individual’s choices, goals, and preferences, while maintaining the highest standards for confidentiality.

Job Summary:

Under supervision of Registered Nurse (RN), provides general direct nursing care to adults with intellectual and developmental disabilities. You will provide prescribed medical treatment and personal care services to individuals in a CRA (Community Residential Alternative) setting, CLS (Community Living Support) and Day Services. Your responsibilities will include providing a person-centered nursing approach, observing and reporting health-related data, coordinating medical care, documenting nursing care, scheduling medical appointments, providing transportation to and from medical appointments and conducting health-related training for unlicensed personnel. You will be responsible for performing quality assurance checks and advocating for the individuals supported and helping to develop community supports ensuring the department provides services in a non-judgmental, non-discriminatory manner that considers cultural diversity and age appropriateness so that autonomy, rights and dignity are preserved.  This is an integral role in ensuring the individuals receive the best possible care and support to thrive.

 Primary Duties & Responsibilities:

• Provide a person-centered approach to nursing care to people with intellectual and developmental disabilities
• Administer prescribed medications and treatments per physician orders
• Perform nursing procedures (e.g. wound care, tube feeding, catheter care etc.) as required
• Provide first aid and emergency care when necessary
• Monitor and document vital signs and health related data
• Implement health protocols and monitor their effectiveness
• Ensure a Medication Administration Record (MAR) is placed in the MAR book each calendar month in CRA/CLS and Day Services
• Complete Medication Reconciliation as needed
• Manages the medication in compliance with DBHDD storing prescribed medications, labeling, dispensing, discarding/disposal, safety and security of all medications

The “Eight Rights”

Right person- check the name on the order and the individual and include the use of at least two identifiers Right medication- check the medication label against the order Right time- check the frequency and time to be given of the ordered medication and double check that the ordered dose is given at the correct time. Confirm when last dose was given Right dose- includes verification of the physician’s medication order of dosage amount of the medication; with the label on the prescription medication container and the MAR document to ensure all are the same Right route- check the order and appropriateness of route ordered and confirm that the individual can take or receive the medication by the ordered route Right position- the correct anatomical position for the medication method or route to ensure its proper effect, instillation and retention. If needed, individual should be assisted to assume the correct position Right documentation- document the administration/supervision after the ordered medication is given on the MAR; and Right to Refuse Medication- document the refusal and report the refusal to the RN and/or Executive/Assistant Director and/or physician

• Schedule and transport to and from medical appointments for those receiving nursing services
• Coordinate preventative and routine medical care for people supported
• Ensure CRA/CLS receives the required care regarding dental services if they receive nursing services
• Provide nursing assessments limited to the scope of practice of an LPN
• Provide nursing care based off nursing service hours as designated by DBHDD Clinical Team
• Complete daily assessments for changes in condition and update Healthcare Plan (HCP) if there are changes in medical conditions, a fall and/or a seizure etc.
• Follows TGSC Fall Protocol Policy in the event of a fall
• Completes CIR (Critical Incident Report)
• Completes GER (General Equivalent Report)
• Follow guidelines of the Behavior Support Plan (BSP)
• Health Risk Screening Tool (HRST) is updated annually, and as medical changes occur
• Participate in Quality Assurance (QA) activities, health risk screening tool (HRST) monitoring, and follow up
• Communicate with physicians, pharmacist(s), specialists, medical suppliers and Interdisciplinary Team regarding care for individuals
• Educate the people receiving supports about health maintenance and behavioral health diseases
• Facilitate referrals to other healthcare professionals and medical facilities
• Maintain accurate documentation and individual medical records
• Document nursing care provided in THERAP
• Educate staff on healthcare plans and health related training to unlicensed personnel in accordance with LPN scope of practice
• Advocate for the people supported and assist with developing community supports
• Facilitates communication and problem solving across all departments for people in service effectively to appropriate personnel
• Oversee compliance to staff to infection control practice
• Track and report communicable diseases and other reportable conditions as required
• Attend ISP (Individualized Service Plan) meetings
• Track all falls and report to leadership and include reason and measure towards resolution
• Ensure compliance with DBHDD Health & Safety standards, HRFA rules and Individual Service Plans (ISP) requirements
• Participate in internal and external audits, site reviews, and inspections (e.g., DBHDD, HFRA, CQL)
• Maintains confidentiality relative to patient care and TGSC practices in accordance with HIPPA (Health Insurance Portability and Accountability Act)
• Maintains competency by participating in continuing education programs and meets state specific requirements
• Complies with accepted ethical conduct and professional standards of nursing practice as set forth by the American Nurses Association or equivalent nursing organization
• All billable units and working hours must be face to face with the individual receiving nursing services.
• Assist in investigations of healthcare-related incidents and implement corrective actions
• Supervise LPN’s
• Provide risk mitigation to reduce the likelihood or minimize potential clinical, legal and regulatory errors to ensure safety of the people
• Complete scheduling for LPNs to ensure adequate coverage for the organization and nurses hours match waiver and allocations and turn completed schedule into RN for approval
• Facilitate all leave requests forms and turn into RN for approval or denial
• Provide training for new LPNs
• Provide training for LPNs as medical conditions of the people change causing changes in supports
• Provide training for DSPs in areas relevant to medical supports within the expected job skills for a DSP
• May be required to work non-traditional working hours other than 8am-4pm Monday- Friday
• May be required to work weekends and holidays
• May be required to work during emergency situations, including but not limited to hurricanes, tornadoes, severe weather events, natural disasters, and other emergencies, to ensure the health and safety of and continuity of services for people served
• Other duties as assigned by RN and/or Executive and/or Assistant Director

Entry Qualifications:

Vocational/Technical degree from an accredited college AND current Georgia license to practice as a Licensed Practical Nurse.

Preferred Qualifications:

In addition to Entry Qualifications, Preferred Qualifications may be added by the agency.

• Successfully completes orientation within 60-days of hire

Minimum Qualifications:

High School diploma/GED and completion of a LPN Program through an approved Georgia Nursing Board; or Vocational/Technical degree from an accredited college and current Georgia license to practice as a Licensed Practical Nurse and two (2) years of experience in a team lead, supervisory or leadership role; or one (1) year experience required at the lower level Licensed Practical Nurse 3 (HCT032) or position equivalent.
