Schedule: Up to 40 hours/week – scheduled within a 7 ‑day work period
Salary Range: $28,571.29 – $36,023.24 – $62,000.00
Starting salary is commensurate with education and relevant experience. Advancement within the pay scale is based on demonstrated performance, increased responsibility, and organizational budget capacity.

The LPN 3 plays a vital role in fulfilling this mission and vision by leading and supporting CRA (Community Residential Alternative) setting, CLS (Community Living Support), and Day Services by providing a person-centered approach to nursing care for people with intellectual and developmental disabilities. The LPN 3 ensures full compliance with applicable local, state, and federal regulations, including those from DBHDD, DCH, and CMS. This position is essential in maintaining a safe and respectful environment that reflects each individual’s choices, goals, and preferences, while maintaining the highest standards for confidentiality.
Job Summary:
Under supervision of Registered Nurse (RN), provides general direct nursing care to adults with intellectual and developmental disabilities. You will provide prescribed medical treatment and personal care services to individuals in a CRA (Community Residential Alternative) setting, CLS (Community Living Support) and Day Services. Your responsibilities will include providing a person-centered nursing approach, observing and reporting health-related data, coordinating medical care, documenting nursing care, scheduling medical appointments, providing transportation to and from medical appointments and conducting health-related training for unlicensed personnel. You will be responsible for performing quality assurance checks and advocating for the individuals supported and helping to develop community supports ensuring the department provides services in a non-judgmental, non-discriminatory manner that considers cultural diversity and age appropriateness so that autonomy, rights and dignity are preserved. This is an integral role in ensuring the individuals receive the best possible care and support to thrive.
Primary Duties & Responsibilities:
The “Eight Rights”
Right person- check the name on the order and the individual and include the use of at least two identifiers Right medication- check the medication label against the order Right time- check the frequency and time to be given of the ordered medication and double check that the ordered dose is given at the correct time. Confirm when last dose was given Right dose- includes verification of the physician’s medication order of dosage amount of the medication; with the label on the prescription medication container and the MAR document to ensure all are the same Right route- check the order and appropriateness of route ordered and confirm that the individual can take or receive the medication by the ordered route Right position- the correct anatomical position for the medication method or route to ensure its proper effect, instillation and retention. If needed, individual should be assisted to assume the correct position Right documentation- document the administration/supervision after the ordered medication is given on the MAR; and Right to Refuse Medication- document the refusal and report the refusal to the RN and/or Executive/Assistant Director and/or physician
Entry Qualifications:
Vocational/Technical degree from an accredited college AND current Georgia license to practice as a Licensed Practical Nurse.
Preferred Qualifications:
In addition to Entry Qualifications, Preferred Qualifications may be added by the agency.

Minimum Qualifications:
