Job Description
A nonprofit services organization in New York City is seeking a Licensed Practical Nurse (LPN) to support resident health and safety in a group home setting through hands-on nursing care, documentation, medication administration, and collaboration with the care team.
About the Opportunity:
Schedule: Monday to Friday
Hours: 9am to 5pm
Setting: Group home
Responsibilities:
- Provide nursing care and monitoring for residents in a group home setting
- Administer medications and treatments in accordance with care plans and nursing standards
- Document clinical observations, medication records, and care activities accurately
- Support coordination of medical appointments, follow-up care, and transitions from hospital care
- Collaborate with RNs, nurse supervisors, direct support professionals, providers, pharmacies, and families as appropriate
Qualifications:
- 1+ year of Nursing
- Active NYS Licensed Practical Nurse (LPN) license
- Experience working with individuals with Intellectual and Developmental Disabilities
- Ability to work collaboratively with interdisciplinary care teams
Desired Skills:
- Knowledge of OPWDD regulations and residential healthcare services
- Experience supporting medication administration standards and infection control practices
- Experience training direct support professionals on delegated health-related tasks
