Park Manor Health & Rehabilitation is seeking a compassionate and organized Licensed Social Worker to join our interdisciplinary care team.

This position plays an important role in supporting residents and families, addressing psychosocial needs, coordinating services, and assisting with discharge planning in our skilled nursing and rehabilitation facility.

Responsibilities

• Assess residents' psychosocial needs and develop individualized care plans.
• Provide emotional support, guidance, and resources to residents and families.
• Coordinate discharge planning and community services.
• Participate in care plan meetings and interdisciplinary team collaboration.
• Assist residents with adjustment, social concerns, and family needs.
• Advocate for resident rights and assist with grievances and advance directives.
• Maintain accurate documentation and ensure regulatory compliance.

Qualifications

• Current Alabama Social Work License – REQUIRED.
• Degree in Social Work from an accredited program.
• Skilled nursing, rehabilitation, long-term care, or healthcare social work experience preferred.
• Knowledge of discharge planning, community resources, and resident rights.
• Strong communication, organizational, and problem-solving skills.
• Compassionate, dependable, and committed to resident-centered care.
