About the Role:
Uplift and Inspire is looking for a passionate and dedicated Life Coach (Direct Support Professional) to join our team in Chalfont, PA. This is a meaningful opportunity to make a real difference by providing one-on-one support, encouragement, and guidance to an individual building independence at home and in the community. Schedule: Monday, Wednesday, and Friday, 11 a.m.–3 p.m. — 12 hours per week
Responsibilities:
- Provide person-centered, one-on-one in-home and community support to an individual with disabilities.
- Support daily living skills such as meal preparation, household tasks, organization, time management, shopping, and money management.
- Follow individualized support plans to promote independence, confidence, and personal growth.
- Offer encouragement, mentorship, and life coaching to help the participant work toward personal goals.
- Facilitate community activities, social connections, and opportunities to explore interests.
- Document participant progress, incidents, and daily activities accurately and in a timely manner.
- Collaborate with the participant, family, and support team while respecting the participant’s choices, preferences, and pace.
Requirements:
- High school diploma or GED required; degree in human services, psychology, or a related field preferred.
- Prior experience as a Direct Support Professional (DSP), caregiver, or life coach is a plus.
- Strong interpersonal and communication skills with a compassionate, patient demeanor.
- Ability to pass background checks.
- Valid driver’s license and reliable transportation preferred.
- Availability to consistently work Monday, Wednesday, and Friday from 11 a.m.–3 p.m.
- Comfortable working in a home with a dog.
- Genuine commitment to uplifting and advocating for individuals in your care.
