    Life Skills Coach / Community Support Professional

    Community Living Counselor

    Home of Guiding Hands | San Diego & East County SD
    Part-Time | Flexible Schedule | Community-Based | $18-$20/hour

    Make an impact by helping adults with disabilities build the skills and confidence to live more independently.

    Home of Guiding Hands (HGH) is looking for compassionate, dependable individuals to work one-on-one with adults with intellectual and developmental disabilities throughout the community.

    This is a hands-on, community-based role. Rather than providing traditional caregiving, you'll teach, coach, encourage, and support individuals as they work toward their personal goals and greater independence.

    What You'll Do

    Every client's goals are different, which means every day can look a little different. You'll meet with individuals in their homes and throughout the community and help them develop practical skills they can use in everyday life.

    You may help someone:

    • Learn how to budget, pay bills, and manage their money
    • Plan meals, grocery shop, cook, and prepare food
    • Schedule and access medical and dental appointments
    • Learn laundry, cleaning, and household management
    • Understand and navigate government benefits and community resources
    • Build healthy relationships, boundaries, and personal safety skills
    • Become more comfortable and confident navigating their community
    • Work toward individualized goals that increase their independence

    You'll Also Be Responsible For

    • Scheduling, planning, and conducting visits with your assigned clients
    • Working with each individual to develop meaningful goals based on their needs and interests
    • Helping clients make progress toward their Individual Service Plan (ISP)
    • Preparing and updating semiannual ISP reports
    • Scheduling and attending ISP meetings and advocating for the individual's needs, preferences, and goals
    • Maintaining accurate and timely client documentation
    • Communicating with families, service coordinators, and other members of the interdisciplinary team
    • Traveling throughout San Diego or Imperial County to meet clients in their homes and other community locations

    Who Would Be Great in This Role?

    You don't need to have all the answers. We're looking for someone who is patient, encouraging, dependable, and genuinely enjoys helping others succeed.

    If you're the type of person who enjoys teaching someone how to do something rather than simply doing it for them, this could be a great fit.

    Qualifications

    • High school diploma or GED required
    • Must be at least 18 years old
    • Experience supporting individuals with intellectual or developmental disabilities is preferred, but not required
    • Strong communication and interpersonal skills
    • Ability to work independently and manage your schedule and client responsibilities
    • Comfortable completing required documentation and using email, online systems, and mobile applications
    • Knowledge of community resources is a plus
    • Valid California driver's license with a safe driving record
    • Reliable transportation and required automobile insurance
    • Ability to travel throughout your assigned service area
    • Ability to meet required background clearance and training requirements
