Job Responsibilities

• Perform comprehensive patient assessments, including OASIS evaluations.
• Develop individualized treatment plans to improve patient mobility, function, independence, and safety.
• Provide physical therapy treatments and educate patients and caregivers.
• Monitor patient progress and update treatment plans as needed.
• Coordinate patient care with physicians, nurses, caregivers, and other healthcare professionals.
• Supervise Physical Therapy Assistants (PTAs) and support staff as required.
• Maintain accurate clinical documentation, OASIS records, and billing paperwork.
• Coordinate patient discharges, transfers, and follow-up care.
• Follow state and federal regulations, agency policies, and physical therapy standards.

Requirements

• Minimum 2 years of physical therapy experience.
• Experience in home health is relevant to this position.
• Ability to perform patient assessments and develop individualized care plans.
• Strong communication, documentation, and patient-care skills.
• Appropriate state licensure required upon acceptance.
