We are seeking a full-time Social Worker to join our Social Services team. In this role, you will advocate for residents while supporting their emotional, psychosocial, and medically related needs. Working closely with residents, families, and the interdisciplinary care team, you will help ensure residents receive individualized support, their rights are respected, and they experience the highest quality of life throughout their stay.
What You'll Do
Advocate for residents' rights, dignity, and emotional well-being.
Complete psychosocial assessments and develop individualized care plans.
Support residents and families through admissions, care planning, and discharge planning.
Coordinate family meetings and collaborate with the interdisciplinary care team.
Connect residents with community resources and needed services.
Address resident concerns and grievances while promoting person-centered care.
Maintain accurate documentation and ensure compliance with state and federal regulations.
What We're Looking For
Bachelor's degree in Social Work or a related field required.
Minimum of 3 years of supervised social work experience in a healthcare setting.
Strong communication, organization, and relationship-building skills.
Knowledge of discharge planning, advance directives, and long-term care regulations preferred.
Supervisory experience is a plus.
