Our 24 bed hospital-based Inpatient Rehabilitation Unit provides post-acute, intensive rehabilitation care for patients with complex medical conditions and injuries. Our patients may be recovering from stroke, spinal cord injury, traumatic brain injury, multi-trauma, neurological conditions, and other complex disease processes that have resulted in a significant change in their functional abilities.
What You’ll Do
- Provide comprehensive nursing care to patients with complex medical, neurological, and traumatic conditions in a post-acute rehabilitation setting.
- Care for patients recovering from stroke, spinal cord injury, brain injury, multi-trauma, and other complex illnesses and injuries.
- Monitor and manage complex medical conditions while supporting patients through the rehabilitation process.
- Promote patient independence by incorporating rehabilitation principles into daily nursing care.
- Collaborate closely with physical, occupational, and speech therapists, physicians, case managers, social workers, and other members of the interdisciplinary team.
- Participate in interdisciplinary care planning and communicate patient progress, barriers, and goals.
- Provide education and support to patients and families to promote successful recovery and transition to the next level of care.
- Recognize changes in patient condition and respond appropriately to medical and rehabilitation needs.
- Support patients in developing the skills, confidence, and independence needed to return home or transition to their next level of care.
- Advocate for patients and actively participate in achieving individualized rehabilitation goals.
- Contribute to a positive, collaborative team environment focused on quality, safety, and patient-centered care.
Experience in rehabilitation nursing is valued, but a passion for learning and a commitment to patient-centered care are what matter most. Specialty certifications are available and encouraged.

Responsibilities:
- Deliver hands-on patient care within scope of practice, including administering medications, supporting comfort needs, and assisting with treatments and interventions.
- Contribute to admission assessments by collecting and documenting patient data, and collaborate with RNs and the care team to implement care plans, monitor progress, and support safe, high-quality outcomes.
- Provide input during rounding and communicate changes in patient condition promptly to ensure timely clinical response.
- Support safe care transitions by maintaining accurate, real-time documentation, participating in hand-offs, and communicating patient progress with the healthcare team.
- Collaborate with RNs and providers to update care plans and ensure continuity of care across the hospitalization.
Qualifications:
Education:
- Graduate of accredited LPN program required.
Experience:
- One (1) year of previous CNA/PCT experience preferred.
License(s)/Certification(s):
- Valid LPN license in the state(s) of practive including eligibility under an NLC multistate license required.
- Iowa Board of Nursing (IABON)
- Illinois Department of Financial & Professional Regulation (IDFPR)
- Wisconsin Department of Safety and Professional Services (WIDSPS)
- Dual licensed positions: Team members must obtain and submit proof of dual licensure within 90 days of hire/transfer. Proof of application is acceptable during this period – required within 90 days of hire.
- Must possess and maintain current Basic Life Support (BLS) certification through the American Heart Association required.
- National Institute of Health Stroke Scale-English preferred:
- National Institute of Health (NIH) Stroke International
- Advanced De-escalation Training preferred:
- Management of Aggressive Behavior Training International Inc (MOAB)
- Crisis Prevention Institute (CPI)
- Uniform Data Systems Med-Rehab for Rehab units (UDSMR)
- IA Child Abuse Mandatory Reporter Training in state(s) where providing care required.
- IL Child Abuse Mandatory Reporter Training in state(s) where providing care required.
- IA Adult Dependent Abuse Mandatory Reporter Training in state(s) where providing care required.
- IL Adult Dependent Abuse Mandatory Reporter Training in state(s) where providing care required.
