Job Summary The Licensed Practical Nurse (LPN) Case Manager is responsible for coordinating and managing patient care under the supervision of a Registered Nurse (RN) and/or Director of Nursing. The LPN Case Manager ensures continuity of care, compliance with physician orders, timely service delivery, and accurate documentation while serving as a liaison between patients, families, caregivers, physicians, and the interdisciplinary care team. Essential Duties and Responsibilities

Coordinate and manage assigned patient caseloads under RN supervision. Implement and monitor physician-ordered plans of care and report patient status changes promptly. Perform ongoing patient assessments within LPN scope of practice. Provide medication management, treatment administration, and patient education as ordered. Ensure timely initiation and continuation of services (SN, PT, OT, ST, HHA). Communicate effectively with physicians regarding patient progress, concerns, and new orders. Coordinate interdisciplinary care and participate in case conferences as required. Monitor compliance with care plans, visit frequencies, and regulatory requirements. Educate patients and caregivers on disease processes, medications, safety, and care plans. Identify patient needs, barriers to care, and opportunities for improvement. Ensure documentation is complete, accurate, and submitted timely in the EMR system. Support quality assurance, infection control, and risk-management initiatives. Assist with discharge planning and care transitions as appropriate. Maintain patient confidentiality and comply with HIPAA regulations at all times.

Documentation & Compliance

Complete all clinical documentation in accordance with agency policies, Medicare/Medicaid, ACHC, and state regulations. Ensure accurate visit notes, care coordination records, communication notes, and follow-up documentation. Participate in audits, chart reviews, and corrective action plans as assigned.
