Contract Length: 13 Weeks (05/12/2026 – 08/15/2026)
Schedule: Part-Time, 2x12-hour shifts/week (alt: 2x8s one week / 3x8s following week)
Shift Times: Evening/Night rotation (e.g., 3:00 PM – 3:30 AM or 7:00 PM – 7:30 AM)
Hours: 24 hrs/week (approx.)
Pay: $1,063 – $1,204/week
Requirements: Every Other Weekend (EOW) + Holidays required
Orientation: Must be available for full-day orientation on Tuesday or Thursday
Provide skilled nursing care to adult/geriatric residents under RN supervision
Assess, monitor, and report physical/psychosocial status changes
Administer medications/treatments; support ADLs per care plan
Contribute to care plans; collaborate with interdisciplinary team
Maintain documentation, safety, and infection control standards
Active NY LPN Licensure
BLS Certification (AHA or RQI required)
Minimum 1 year LPN experience (LTC preferred)
Ability to care for adult and geriatric populations
Graduate of accredited LPN program
Willingness to work in environment with potential exposure to blood/body fluids
