JOB SUMMARY

The Hospice Scheduler provides scheduling of Hospice visits.

DUTIES AND RESPONSIBILITIES:
  • Answers inquiries from clients, POAs, staff, physicians, and anyone else this position requires in a friendly and cooperative manner. Directs questions that cannot be answered to appropriate person
  • Contacts field staff via email/phone call per direction given by leadership or clients/APOAs
  • Maintains files for the agency
  • Handles client referrals as needed for hospice services and initiates all referrals for hospice
  • Schedules SOCs and communicates with clinicians via email/phone
  • Performs all hospice workflow scheduling for the agency including changing codes for clinicians
  • Schedules supervisory visits
  • Requests forms as needed
  • Performs other duties as required from supervisor.

JOB REQUIREMENTS (EDUCATION, EXPERIENCE, KNOWLEDGE, SKILLS & ABILITIES):

Must have completed a requisite practical nursing program or be licensed in the state where the agency is located.

Have three years nursing experience preferred. Home Health and/or hospice experience preferred.

Currently licensed as a Licensed Practical Nurse in the state.

Complies with accepted professional standards and practice.

Demonstrates good verbal and written communication and organization skills.

Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.

Working knowledge of computer systems

Excellent coordination, communication, negotiation, and public relation skills.

Detail oriented and able to work with minimal supervision

Demonstrates ability to protect the confidentiality of patient and organization information
