Taylor Residences provides have proudly served Jacksonville’s seniors for over 50 years.

Currently, we are looking for Licensed Practical (LPN )to be an integral part of a team that helps change the lives of our residents each day and where you can make a profound difference by applying your nursing expertise at our Taylor Care Center Facility.

The LPN role provides care and treatment in accordance with physicians’ orders. Works in a team environment and within scope of practice as defined by the State. The Licensed Practical Nurse works under the direction of physicians and Registered Nurses.

Essential Job Function:

• Assesses patients by physical examination, including pertinent diagnostic testing to determine health status.
• Administers medications and treatments.
• Participates in the care planning process.
• Supervises personal-care workers and certified nursing assistants, monitoring assigned personnel to assure they are following established standards of care, regulations, and appropriate use of equipment and supplies.
• Communicates with physicians regarding changes in residence condition, diagnostic test results, etc.
• Documents assessments and care in compliance with standards of care and company policy
• Educates patients and their families on health related issues.
• Completes accident/incident reports as necessary with proper documentation.
• Informs personnel of new admissions, diagnoses, and room assignments.
• Participates in all required training.
• Performs other duties as assigned.

Open Shifts:

Full Time , Part-Time or PRN

• 6:45am-3:15pm
• 2:45pm-11:15pm
• 10:45pm-7:15am

QUALIFICATIONS

• Graduate of an accredited school of nursing.
• Valid Florida State license as an LPN.
• Current CPR certification..
• Minimum 2 years’ experience in adult living community or skilled nursing facility preferred.
• Be able to meet all AHCA regulations and pass pre-employment physical exam. This requirement also includes drug screening Level II background screen and reference inquiry.
